A step-by-step guide to the use of OUFAU's service for processing Disclosure applications If you wish to use our services then please follow these steps carefully: • Write to us on your organisation’s official letterhead requesting that we consider providing such a service to your organisation. This letter must indicate that your organisation agrees to abide by the CRB’s Code of Practice and must bear the live signature of an authorised individual (eg: a manager or director). • Enclose a detailed job description of the position(s) for which you are seeking a Disclosure. A separate page is required for each position, and should indicate which under application code (refer to leaflet DIP 003) you are proposing to make a Disclosure application. • Enclose your organisation’s policy statements on: • Enclose a cheque made payable to “Oxford University First Aid Unit” for £50.00. • Send the covering letter, job description(s), policy statements, and cheque
to: Upon receipt of the above, we will make a decision on whether we can extend our services to your organisation and inform you by e-mail. If we are able to assist you in this manner, we will write back inviting you to send in payment of £15.00 per Disclosure application to cover our application processing fees. Upon receipt of this payment we will send out the appropriate number of application forms to you by post together with a purple booklet containing guidance notes on their completion. Once pages 1 – 3 of the form(s) are complete, e-mail us to arrange a time to meet. This meeting will be at a mutually convenient time (which may have to be outside normal ‘office hours’) and held either at our office in Central Oxford or at any other location where our Countersignatory is working. Should you request that our Countersignatory visits your location then further charges will apply to cover travel time and expenses. Our Countersignatory does not need to meet each applicant, but will need to see several pieces of original ID (as specified in DIP 002 and the purple guidance notes booklet) for each individual in order to complete page 4 of the form. Please note that we are not permitted to accept photocopies of documentation, nor may we accept other forms of ID not listed. Under no circumstances should completed application forms and original ID be sent to our office as we are unable to guarantee their safety in transit to us or to offer storage facilities. Once the form has been countersigned, we will send it to the CRB for processing: a Disclosure would then normally be issued within 2 – 4 weeks. At all times, client organisations and Disclosure applicants must comply with reasonable requests for information from OUFAU, the CRB, or either of their agents. Failure to do so in a timely manner may prevent the processing of an application and any fees would be forfeit. |
